Update: I tried running it in windows mode, turning off v-sync and anti-aliasing, and it still crashes.
I also monitored my CPU and GPU temp during my last play session (which lasted about 5 minutes), and nothing was crazy (my CPU was at 61C the last time it CTD).
I can marathon Far Cry 4 on high settings with no problems, so I can't imagine this is a hardware issue. In my years of PC gaming, I've never had a game just consistently CTD with no errors like this, so I'm at a loss.
Any help, or acknowledgment by EA, would be greatly appreciated! Thanks!
