If you are locked out of the start menu, go to
Users\user\Documents\BioWare\Dragon Age Inquisition\Save
Delete the preferences files there.
Still doesn't fix gamepad support but will let you play w/ M&KB.
I have Windows 8.1 x64, pre-loaded DA:I, plugged in hardwired 360 controller. Windows had a driver built in, no extra downloads or installs needed. Launched game, changed controls to gamepad, worked flawlessly. So it's not a universal problem.

I do have a Razer mouse and Razer Nostromo onboard, next time I get a chance to play I'll try deplugging all my other USBs (but this is a huge pain that still qualifies as a bug) - 😉
I am having this issue and I have no Razr hardware, no other controllers, no special drivers, nothing. I am running a stock Windows 8.1 install with only a 360 wired controller, logitech G700 mouse, and logitech K800 keyboard. I switch on gamepad controls and all input to the game goes completely dead.
Other games still work perfectly fine with the gamepad.
Same with me. I have nothing special hooked up besides my keyboard, mouse, and 360 wired controller. I have tried unplugging the keyboard and mouse, clean booting, updating drivers, and still nothing--this is mildly frustrating and I hope for a fix in a reasonable amount of time.
**Sucess - I disconnected my Nostromo and gamepad is working!
However, I must insist that this is NOT a bug with the Razer Nostromo or an issue with Razer hardware or software...because I am able to keep my Nostromo plugged in and working with the Gamepad on many many other games. Dragon Age Inquisition and Dark Souls are basically the only 2 games that I can't use gamepad while Nostromo is plugged in.
For reference...while I don't think EA Moderator comments were intended to offend, I would like to provide some customer service / linguistic feedback. When you use the phrase "to see if Razer hardware may be the source of the issue" - you are attributing fault to the Razer devices which are working 100% fine with many other pieces of software. Clearly, the issue lies with Dragon Age Inquisition - you will get a better response from your customers if you just admit when there is a problem on your side and you are working to fix it. Language like this implies that the problem is on our side....when you have no evidence to suggest such, especially based on the plethora of reports.
Not mad, just sayin - I want you guys to shed the image of crappy customer service, because I do think you're trying.
Hello, I had the same problem. I have Razer Nostromo controller and the game doesn't detect the Xbox360 gamepad.
If unplug Nostromo the game works normally.
For now I'm playing without Nostromo Controller.
Please EA fix the issue as soon as possible.
