I'm having the same problem, on PC. I judged her before speaking to her and now she's nowhere to be found.
I'm furious about it, specially because I'm taking my time to complete everything on my first playthrough.

Same problem.
I visited Skyhold mid-quest to sell off some goods and to send out some missions from the War Room. As I passed the throne, I got to sentencePoulin, which I did. Now I am unable to complete the quest.

Surprises me this bug wasn't cought in the betas.
