This is happening to me also along with a bunch of other quests which wont register or turn in.
Emprise Du Lion - Quarry Quandary : Mistress Poulin is nowhere in Skyhold or her town.
Crestwood - Wyrm Hole : No Wyverns loading in the cave.
The Western Approach - Draconology : Will not let me turn in quest but let me aquire Frederic as an agent.
Wartable: A present for Bianca : popped up as in progress on my table and hasn't budged since it says that leliana is doing it but i can still assign her to other things.
Companion Quests - Dorian : Dorian's second quest wont start from what I have read Giselle is supposed to come to me in the main hall with a letter, she has never left the garden.
Those are the quests ive found that are bugged for ps4 anyway. I have completed the entire war table and have only these few quests to complete and this 1 character quest line to finish before I go face Corypheus, as a completionist I would like to finish them before finishing the game. I hope this helps someone and if anyone knows a fix or work around, by all means, let me know.
Mine are different for PS4. Poulin bug yes. Iwas able to complete Dorian's quest, and my quest completed bubble is filled but it won't clear. Still says to talk to fancypants whatshisname. I didn't have an issue with your three middle bugs. Maybe those are event based?
