I have the same problem, I judged her in skyhold before turning in the quest and she disappeared.

  Having the same issue on PS4. After completing the mission went to the location to hand it in. She was there but I could not hand the mission In. But I could complete two others which where in the same location. Travelled to skyhold, judge her, went back to the location where you could hand in the mission and she has disapeared....just a diamond on the map but no person. Am about 3 hours from my last save, and really don't want to have to reload and have to do everything again!

    Finished up in the quarry. Went to skyhold and noticed I had a new judgement. Judged Poulin to scribing for the inquisition. Later noticed I had a quest to talk to her about the quarry.
    Unable to complete the quest: Quarry Quandry - Speak with mistress Poulin
