PS4 Dragon Age Inquisition GotY Edition won't play after redeeming codes

After installing 1.12 and starting a new game, I made a save file. Then I exited the game and redeemed the DLC codes. After redeeming the codes, the game won't launch. I tried initializing the console...

I have this same problem. Brand new PS4 Pro, brand new Dragon Age Inquisition Game of the Year Edition. 

Game will load without DLC. Once DLC is installed, game remains on black loading screen with Inquisition logo in lower right corner and will no longer start.
