This is also an issue on mechanical hard drives, which is odd, because the current gen consoles have slow 5400RPM drives and as far as I know, they don't have this issue.

I used to have the issue where NPC wouldn't load and the workaround was to walk away and come back. (The NPCs don't load while you're looking.) The issue got better when I upgraded to a better graphics card and then disappeared completely when I moved the game from a standard 7200RPM hard drive to an SSD.
