I also encountered this bug, but it was immediately after picking up the new rift power on the island with the weeping lady. When I was on that Island I was unable to fast travel anywhere on the map. Once i took the boat back I was able to fast travel again (after several reloads). However on the island and after I experiences CTDs when I tried load saves or even tryingto save games.
Everything was fine....up until I got the new power. Related I don't know.
Prior to this the new mandatory patch was applied, as well as Black Emprium and the Multiplayer DLC.
Boy....Good thing Witcher 3 is almost here.....
