Im in the same situation you are my game still think that its installing can't load old game files can't skip cutscense how did you fix your issues?
Actually just as I said above: I went into the system storage and looked to see the status of the Dragon Age - it showed it had only installed 26 gig of 41 gig total. So I left my PS4 on a few hours and let it fully install.
If you're not sure where the system storage is - you have to go into the PS4 settings (profile; storage; time - they're all under one of the options) - then go do to storage managment (or console management - I don't remember what it's called - but all games you have on your system will be listed) - find dragon age - get details and you'll see it say '28.44g of 41.5g' (or such) - once it's completely done, it'll say 'complete' (If I remember right).
I'm not sure sitting on the main screen helped any - and I know being in game did not. So that's why I sat on that screen and waiting until it was done - and I've not had any problems since.
