Repairing tha game may fix it for you. I'd give that a try first and hope it works.
The alternative is Cheat Engine with the DAI 45 Table. Most of it is iffy or doesn't work anymore but, the Inquisition Editor does and, with that you can give yourself the lost shards. (did that to me once on one in the Hinterlands.)
Assuming you are on a PC, can't use cheat Engine on consoles.
