@angus192711 ;  Wrong forum (you posted in the DA:O one) but it means the item is less powerful than the one the currently selected character has equipped.  3-stars = same(-ish), and 4-or-5 are better.

Personally, I don't find those ratings very accurate. But maybe that's just me...
