@thosrtanner You haven't been accidentally hitting the 'mark all entries as read' button, by any chance?
Some of the codex sections are quite large, especially if you have DLC installed, so you do need to scroll down quite far on some of them, perhaps you're missing some further down? I know on mine, every single playthrough, there's one codex section (I think it's tales) which gets stuck in the 80's when I scroll, I have to use my mouse to unstick it to continue scrolling down (it goes well into the 100s, if I remember correctly).

  • Yeah, I know the sticky codex section very well.

    No, I'm not marking all read (I haven't found the button. i wish I had - as its a nightmare spotting that yellow star sometimes!). I will try to get some more definite evidence as it worked just now.

  • OK, I have picked up a few codex entries during exploring the hissing wastes. Apparently monster updates are OK and the inscriptions from the dwarven tombs. But I found the 'The First Blight Chapter 3' and read it. And it appears that it got into my codex but somehow got marked as read - my history codex didn't have a star by it, and the codex entry didn't have a star on it. But it does appear to be there.
