You broke the doors at all three locations before claiming the keeps? If not then that's how you lost one. Even if you enter the keep another way, you need to break the door to get the sad splinters and, that has to be done before you claim the keep.

I believe that is only part of the problem DarkAmaranth1966. I did receive sad splinters by destroying the doors as the first act to entering the keep. But I only have 2 of them as well. I think another part of the problem is that you don't receive the splinters until you have the schematic. I didn't receive the schematic until after taking the Crestwood keep and that is the only door that did not drop splinters for me.
But maybe that was just my game, as I haven't really looked to see if that is true for anyone else.
No, You get sad splinters even if you do not have the schematic. The only possible way to lose them after getting them would be to sell them, which could be done accidentally prior to the last patch but, not after.

More than once (across my many, many play-throughs) I've forgotten to look for/pick-up the splinter after breaking a keep's doors. My own fault, and they do disappear after an interval. <facepalm>
