So.. I switched from 1920*1080 at 144 Hz to same resolution on 60 Hz. Result? http://imgur.com/ma4xWze You can see it is still Full HD by the crisp sharp character portraits but everything else has gone to **bleep**. This was the only change I did, these are still the Ultra settings. I guess this game needs a lot of work in regards to fixing these glitches. :-( Im sad.
Good lord. Since we seem to be the only two people on the internet having this problem right now (I have searched everywhere!), we should compare.
We both have:
-Win 8.1 x64
-Nvidia GTX series cards
-Latest graphics drivers
Which lots of other people have, yet apparently no one else is having this problem, or at least they're not posting about it.
I'm running DA:I off my HDD, but Origin is installed to my SSD, along with my OS. Is your game installed on the same drive as Origin and your OS? I doubt it's the cause of the problem, but I've had issues running games on a drive that's seperate from my OS before, so who knows?
At this point I think I'm just going to re-install the game and see if that magically fixes the problem, since I'm all out of ideas. ☹️

Game is installed on a seperate drive.
Far Cry 4 is looking absolutely stunning so I doubt it´s the driver itself.
Nothing more to compare here, really. :-(
Im now disabling Cloud Storage and delete my delete/backup my saves and config and let the game create that again without restoring it out of the cloud. let´s see if that changes anything.

Well I tried uninstalling and reinstalling with no luck...still black grass!

Completely new save didnt help either. Made some new screenshot though: http://imgur.com/a/FMkBB These squares are literally everywhere.
