I have exact same problem, worst cave is the "Pirate cave" above small grove camp. There is a tiny elcove half way through cave that has a small hole in the wall. All my companions head straight to it and try to attack through the small hole in the wall. If i tactically move them out they teleport back into elcove when i release tactics to battle spiders. Very frustrating! Took me 7 minutes using tactical camera mode to move them back out of cave where they would respone as a team again and not try to run to the elcove.
What platform is this? PC or console, and if console which one?
In my case happens that ranged sidekick sometimes can Lydia* the door and prevent others to enter, but it's not just these caves and it's not actually AI that got stuck. By moving the sidekick a bit further they all enter.
