I'm having the same issue with the graveyard tomb on PS4. Got the order wrong and one of the torches didn't extinguish. I killed the guardians but the torch stayed on and once the other torches were lit the door wouldn't open. Now the torches are all lit but the door still won't budge.I'm on PC having the same issue as well. I'm going to try the one solution of killing everything I can within the area, but I just wanted to let you know I had the same issue, and wonder if there had been any solutions so far.
Update - Killing everything did not work; nor did deleting most of my saves (except the most current one in Skyhold) going back to before I entered the Hissing Wastes
