Unable to change difficulty of game (it is locked)

I have been playing through DA: Inquisition at a leisurely pace, mostly on the hard difficulty as to not make it too easy for me.  However, there were two points I changed the difficulty.  The one I remember clearly was after the grand ball when I faced that really tough lady...  My potions were down to nothing and the saves I had were before going on that mission or the autosave, which put me right back to the fight with that lady, potionless and helpless.  Anyway, I changed the difficulty down to normal, or possibly even easy? to beat her.  

Once she was dead, I either kept it at normal, or if I changed it to easy, I changed it back to normal.  

No problems...  I was going through questing a few weeks later and noticed that I was having a really easy time of it, so I wanted to ramp up the difficulty again to hard, but the indicator seems locked.  It is set at normal and I cannot hit the green A (select) button to change the combat difficulty.  I can choose B (back), and Y (reset), which I thought reset might work, but that did not do much.  

I am wondering if it is tied into an achievement system or something that now I am stuck on Normal for the rest of the game?  I got too overpowered and now I am killing enemies like it is nothing.  I don't even think about Fade Rifts anymore, I just point and shoot til everything is dead.  

Is there any way to change the gameplay back to hard?  I can scroll through all the difficulties, but none of them are able to be selected.  I tried doing it out of combat, and then when that did not work, in combat.  Maybe I have to be at Skyhold to do it?  I will try there, but so far nothing has worked.  Imma go try Skyhold, and if that works I will update here...  If not, someone please help!

    Nevermind, I am an idiot.  I just solved this.  

    The A (accept) button does not need to be highlighted for me to accept.  I just scroll to the difficulty I want, in this case hard, and then hit the B (back) button and agree to the changes.  Done.  

    I must have done this on the other occasions, but not realized it.  Sorry for wasting your time, guys.

    Still locked, even at Skyhold.  Not sure why this is happening.  

    Please help.
