Okay, a few things:
- Did it start directly after rebooting the PS4 after installing the Nyko Data Bank? Or only some time after? If it's directly after, you might want to consider backing up your data, removing the Data Bank, check for any visible physical damage on both your PS4 and Nyko DB, and carefully place back the Nyko DB.
- Error CE-38612-0. I just googled it, and it has to do with the Sony DB. What is recommended is to reboot the PS4 into Safe Mode and allow it to rebuild the databases. https://support.us.playstation.com/articles/en_US/KC_Article/Safe-Mode-PS4/
- It seems CE-38612-0 is for some a persistent error which started to happen months ago. Some have temporary fixed it by disabling the 'forced' internet connection on the PS4, but that creates the issue you cannot retrieve patches (you'll have to make single boots where you only retrieve updates etc, shut down and then unplug the connection again so you can play again. Sony apparently hasn't fixed this particular issue yet.
