I encounter same issues in Xbox one,
But it's seem this never occur in PC early version(patch 4 or early), and I don't know whether it exist in PS4/PS3/XBOX360 version.
it's seems Devs completely ignore this massive bug, it still exist after patch 11. I guess such bug never fixed because no further patch for this game.
Currently I got three way to fix this.
1. Quit tactic mode and press bottun "A"
2. Quit tactic mode and press RT
3. Disengage
For deep gamer and tactic mode beloved, this bug was completely unacceptable.
I have the same issue, unfortunately.
What I found to work, is when I spot my char from Tac Cam freezing, to just make them use an ability. Seems to do the trick while staying in Tac Cam.
