Ok I found a solution, first before trying this, if you have mods disable them (including cosmetic ones like reshaders, anything that alters the game files), otherwise do a fresh install of the game ...

Just slow down the damage and WAIT - when he is up the hill just do nothing and wait until he comes down again - both times. overly damage leads to the bug again :)
