Graphics reset on start up

I run my settings at ultra/high depending on what zone I am in (Arlathan lags bad for me so I lower it to high) but since the last patch when I load up my game all of my settings are set to medium/low.  It is frustrating because it sets my level of detail to low but I can't reset it to high unless I restart... but if I restart it gets set back to low again.  Is there any way to fix this?  I tried setting the graphics at the title screen, but that doesn't do any good because I still need to restart.  I would prefer not to set my preferences via the Nvidia app.

My comp specs are below.  Hopefully I have the right info, no idea if I got it all or not.  If there is something else needed please let me know.
