Hi, adding my log.
I have been able to play the game with no issues on Windowed mode, even at the highest resolution my monitor can handle. As soon as I maximize the screen, it crashes.

Mainboard name not in the dxdiag thanks, please confirm.
Also, your resolution in-game is set higher than native, try deleting the hardware_settings_config.xml
Downgrade the resolution to 4K (3840x2160) not Super Wide 4K (5160x2160) with the in-game settings. Choose Fullscreen.
Steam wiill let you switch to a window using ALT-ENTER then you can ALT-TAB without issue.
I believe the game uses FSE though I haven't had any confirmation from CM/EA.
