Disconnected from two games consecutively

Garima told me to post this here so that the investigation team can have a look into the games that I was disconnected from in champs despite already winning. 

I have game recordings of this if required. 

please could someone help me out? I also lost out on evos and just wasted 40 mins of my time pretty much.
