I had the same problem. 
My PSN is The_Sherrifs.

Same problem as the person above described. 
The challenges were available for a short period and I completed them, not knowing it was a mistake by EA. 

I hope to see my actual rewards.

Summarize your bug The all madden challenges were available for a short period of time and I completed them. But in my rewards was the same 76 ovr player. Not the rewards I was supposed to receive. Then the challenges were gone. Now they are back but I did not receive the rewards nor can I complete them again for the rewards. As you can see on the pictures, it says I completed all the challenges but I can’t upgrade john madden because I did not receive the rewards

Same here I didn't know it released early in error which now has cost me missing tokens. Here screen shot to  see been done but not checked marked as completed. 

I played the All Madden Challenges the other day which I completed the first 3 before it was removed. I didn't noticed it was added early in error which now I'm missing 3 tokens and rewards from completing them. Here is acreen shot from my challenges it shows was done with no check marks and now 21/18 stars. Are they any way to fix this glitch due to not my fault it was available in challenges to conplete early? Thank you Josh

I am having the same issue
