same issue here. controller didn't work on initial launch, went through all the troubleshooting steps I can find, restart pc, finally works for 2 days. Then it randomly stopped working and nothing will fix it. No amount of messing with steam settings, launched, unlaunched will do anything to get my ps4 controller to work. You'd think a year after launch they could get a controller to work like every other game company that sells me a game on pc.
