H2H - Adjusting defense causes zones to disappear or modify on their own

For a few days now, once every 5 to 8 plays when I adjust my defense, whether activating QB contain, manning up specific players, or reassigning a new zone to a DB, the entire play get’s thrown off. 

For example on a cover 4, I’ll man up the outside receivers and put the safeties in deep halves, or at least I attempt to. All of a sudden I have players assigned to the same zone, some dbs assigned to the opposite side of the field, some dbs assigned to blitz, so when the play snaps, they literally high tail it out of their zone to the other side of the field, etc…

This occurs most often when I use QB contain.  It is definitely a significant glitch and I cannot be the only one who has lost several games for it. Can anyone simply acknowledge that this is a real issue?  Photos attached.
