Superstar mode objectives

I’m in the first year of superstar mode and the objectives for each game often do not get recognized even if they are met while playing the game. I need 3 “3 and outs” but when I get one it doesn’t register. This has happened multiple times for different objectives. 

Also when a player gets injured in game, when they are shown on the bench the helmet is always just a silver blank helmet.
