When you created all of your custom players and added them to a roster, did you save the roster afterwards?

It sounds like the roster wasn't fully saved or there is something going wrong during the save process. Have you tried power cycling your console lately and do you use an external hard drive?

How to power cycle your PS4 console

1. Touch the power button on the front of the PS4 for at least 7 seconds (until the system beeps twice).
2. Turn it back on after 1 minute. Launch the game.
