I am having the same problem that you described. I purchased my Xbox One Madden bundle at midnight on 8/26 from Gamestop.  I have contacted Microsoft support by phone.  They acknowledged that there may be a problem, and told me to wait a couple of days for the fix, and that they would be calling me back with the resolution.  Being hopeful.

More detail below:

I purchased the Madden 15 Xbox One bundle from Amazon and used the provided code to download the full game. I entered the code and was prompted to accept the terms of the game and selected 'Confirm'. 

The system did not prompt me to download the game and when I searched the digital game, was prompted to purchase for full price. 

I tried the download code again and recieved a message relating to issues and to try the code one more time. 

On the third try I received a message that I now owned an Xbox bundle (no mention of which bundle) and tried to search and download the digital copy again only to be prompted again to purchase for full price.

I went ahaead and purchased the EA Access to get in the game and was able to use the trial, but received a prompt that my code was accepted and my Pro Pack bundles 🏀 were wating in the MUT hub. 

The game/system seem to have my game access mixed up.

    Good to know that I'm not the only one. I'm sure the issue gets resolved with a new code, just hoping that the issue is resolved sooner han later.

    Spoke with an EA rep last night and went through the first parts of troubleshooting. Deleted the game and re-downloaded. Looked to be going well until I was notified that "Download is almost complete. Please insert game disc."

    Went over my Origin account and was told that the MUT Ultimate Pack (only available through my d/l code) and was showing and that I had EA access. Rep and I hoped that the EA Access trial download was getting the One 'confused' with my access to the full digital copy. 

    -Cleared cache

    -Deleted alternate Mac address

    -Deleted Gamer tag and signed in again 

    -Deleted game (which showed at the full 14.7GB) and re-intalled to be told that I still needed the game disc.

    I will be calling again today to see what other options can be pursued before having to receive a new download code.
