Go to franchise settings & turn progressive fatigue off, then gameplay settings and fatigue slider down to zero. Last go to manage staff -> auto sub: turn ALL “sub out” sliders all the way to 0 so your starters aren’t subbed out. Last step is what fixed it for me.

my starting 1 receiver kept going out after being fatigued and would never come back from the substitution. I was able to sim one play, and it re-inserted the starter after that. Worked for me so hope it helps you guys.

I'm having a similar issue. Except it's one player. I got to I wanna say week 10 or 11 in my Franchise with the Bills. All of a sudden at first Tredavious White would come out in 4-3 packages, but not in any Nickel or Dime plays. He was always subbed out for my CB4. So I swapped them in the depth chart in game (figuring I'd compromise using my cb4 in all 4-3 sets since I use Nickel most of the game anyway).
But now I'm in week 15, and I can't get White on the field at all. I went back and turned off Progressive Fatigue, went back into the game and it didn't matter. No matter what, he just doesn't come out in the field.
Any idea why? Or how to fix this?

having the same issues, my top 3 receivers and starting RB check out after the first play and never come back in

I love how everyone reports the same issue for over a year and still there is zero response from EA. What a trash company.
