LvL extra point attempt denied by game

The weirdest happened to me a little less then an hour ago. 

In my 2nd drive of our LvL  matchup I scored a TD. However, instead of going to the point after attempt, the game immediately loaded the LvL home screen. Therefore denying me my PAT.

See enclosed/included screenshots in which you can see that the film doesn't show the chosen play in the PAT. In normal cases the chosen play gets shown, when both failed or succesful.
