Are you using Windows 7 (64bit)? I have the same problem, every few weeks or so my single player accomplishments get reset, including (most frustratingly) the ones that unlock the bonus medbay powers. Why they can't be stored server-side like multiplayer achievements I have no idea.
Yup, on Windows 7 x64.
I've noticed similar problems with several games, and I think it has something to do with the way W7 x64 stores files in default folders. The advice I've received with other games is to install somewhere other than program files or program files(x86). Gonna try that myself and see if it fixes anything.
