Achievments not unlocked

Hallo

After years I decided to complete the last achievements of ME on Steam. After my great effort, the achievement Insanity 3 did not unlock. Although I finished the game on the highest difficulty. The previous two unlocked normally. The same with ME1 majority of the game. Also everything is completed. Please, what about this? How can I request a fix? I spent dozens of hours on it and then some bug prevents me from doing it
