There are several people having a problem with the decryption mini game in ME1. The problem is that the mini game is auto failing regardless of whether the correct button is pressed withing the allotted time or not.
I have played ME1 since I bought my first xbox 360 (elite) back in 2008 and have played over 20 characters, so I can say I know how the decryption mini game is supposed to be played. I upgraded to the slim 360 when the MW3 edition came out and had no issues with the game on that console until this last August.
That's when I downloaded the Leviathan update for ME3 (and also received the console updates and other game updates at that time). Since August I've had this issue. While using one of the MW3 controllers the mini game would auto fail on the first button pressed. When I switched to one of my older controllers it was the third button pressed in the sequence. Given that my video was starting to get glitchy while playing ME3 multiplayer I thought the console was going bad. So I recently upgraged to a Halo 4 edition. I also redownloaded all my DLCs rather than transfer the hard drives/transfer the data from the hard drives. However, even on the new console, with the Halo 4 controllers, the mini game is still failing. This time on the 2nd button in the sequence. Note, that this is only for lockers. The computer in the embassy was "hacked" as normal.
I have read about this issue on the bioware forums, and this issue seems to be on the xbox as well as the PS3. I have not found any response on those forums that gives a solution to this issue.

These threads were added so that anyone that wishes to check out the bioware forums to see if I missed the "answer" may do so, or to use it to research an answer. The former thread gives a solution, but that solution isn't to the problem I described. Any assistance on this matter will be greatly appreciated.
