Just completed ME 2 Insanity run near an hour ago, all achievements have been unlocked without any issues (PC, Steam). All content was completed before the omega 4 relay jump.
I am still having the issue one Xbox one X. Completed the game, but didn't get either Paramour II or insanity II. I am on the latest patch.
This is a little frustrating. Good thing I have a save before the omega replay in case they decide to fix this issue.
