I have a similar problem, but with ME3. Completed ME1/ME2 on insanity, was halfway through ME3. Then there was a patch (June, 7), I wonder if it's involved somehow. Then I've finished ME3 and nothing. I even defeated Shepard's clones in Armax Arena on insanity and it all was for nothing.
