I've triggered this twice in the various playthroughs I've done over the years. I don't recall how I fixed it the first time (more than likely I used an older save since I do so constantly). I can't say with 100% assurance how it is triggered but what I did was complete all the mini-tasks in Tuchunka before talking to Wrex: talked to all available NPCs, did pyjak mini-game, feed the varren, bought all available merchandise from both stores, sucker punched the pyjak, ec. I did NOT do Mordin's quest yet. When i spoke to Wrex there was a skip in the cutscene where Wrex welcomed Grunt to Clan Urdot. That's how you know the game is about to break (or rather IS broken) when you attempt to talk to the shaman. Doing Mordin's quest doesn't fix the problem.
Fix: I switched out squadmates, went upstairs to the Shaman, and did a hard save. I loaded up an older save (when I first arrived to Tuchunka by shuttle) then reloaded the newest save again (where I switched out the squadmates). This triggered the cutscene for the shaman to speak. I went back to the shuttle, grabbed Grunt and a squadmate (this case Mordin again), then continued the quest as normal. You may be ale to fix it by simply loading up an older save then reloading the newest one (without having to switch squadmates) but better to side with full disclosure. Hope that helps anyone in the future.
