Mass effect 3 Citadel Bug - Miranda keeps dying

Hi there, I've purchased the Citadel DLC, played up to the Horizon planet to ensure I can have the full party threw Citadel, For some reason Miranda Lawson keeps dying at the end of the Horizon miss...

you have to meet her in citadel 3 times and always choose the "paragon" option, first in docking area after palaven, then in spectre office and later in that apartment. You don't have to romance her in ME2 to save her

Do you have to meet her all 3 times?
I missed her the first time, but caught her the second and third. I warned her about Kai Leng and also gave her alliance support. I talked down her dad too, but she died.
