I'm starting to understand that something is amiss. Sometimes the puzzles are much less forgiving than others, they even 'look' faster. It is possible that the timing is programmed to be different. Some seem easy and others they are impossible, even though I know that I'm hitting the button in time. I was wondering if it could be related to how much charge my controller has. My PS3 is connected to my TV through an HDMI HT receiver, so that is a constant. I got a new controller and I'm going to go back to the ones I skipped and see if they are any different.
I also would test a direct connection to your display or a different display if that is possible.

Yes, I did do that I connected it directly to a monitor (not a TV). It's not my equipment.
It doesn't matter, I got to the end of the game and moved on to ME2. Overall it was a minor issue in a very enjoyable game - that still holds up 10 years later.

i know that this is like 7 years later, but if anyone is having this problem still, I used to ACE those puzzle and never fail any , 7 years of playing this game and not once i failed, until now, and it turns out my new tv set had a huge latency that made my life hell, switching tv to game mode in both picture mode and sound mode helped ALOT! so maybe try playing around in your tv settings
