Both usually happens when ME3 looses connection to the Galaxy at War server. Once the connection has re-established, maybe after a restart everything should be fine.
After the Extended Cut you don't neet the Assets from MP promotions or even a higher readiness than 50% to see every ending anyway, so even if the GaW servers are down, it's not such a big deal.
