Non-unlockable codex entries
As far as I can tell, the two codex entries, "Space Combat: Trans-Relay Assaults" and "Weapons: Javelin", have no way of unlocking. According to the wiki (http://masseffect.wikia.com/wiki/Codex_Entry...

Please keep in mind that not everything that one can find in the code of the game (e.g. by data mining), ends up in the final version of the game.
