Looks like you don't have enough RAM, so OS extremely using swap file.

Community ManagerIn addition to what @s1owp0ke has posted, it is possible your hard drive is close to failure as the read speeds do not really correspond to the expected values at 100% usage.
Either that or your hard drive is spectacularly slow but maybe I am somewhat spoiled by using SSDs at work and home.

I've had a similar issue with GTA V on my old hard drive, which was going on 7 years. I partially fixed it by using a program called CleanMem, which purges the RAM at regular interval and makes transfers a little easier on your system.
However that's not a cure, just a partial treatment. Get a better hard drive or better RAM.
