Extreme difficulty level
Hello, I'd like to ask if there is any way to play the extreme difficulty level without a playthrough first, for a more challenging experience?

It seems not. In general, games (e.g Resident Evil series) require you to complete the campaign mode in much normal difficulty, then increase it.
