NFS Crashes constantly without being able to play (PS4)

Need for speed heat keeps crashing on PS4. 

I tried everything with the ps4 to see if it will help/work, I initialized it, I installed the ps4 UI software from their website to initialize and that didn't even work. I even swapped hdd's and installed it there but to no result, the game would load for 5 seconds then crash and it keeps doing that. if any of you know what to do please help.
