NFS heat

What is wrong with nfs heat every time I go to play my game it either glitches really bad or it says it took to long to load and I gotta restart the console or close the game several times

I can’t remember what exactly it says but I know it says the game or disc took too long to load and when it does it starts glitching like everything will disappear sometimes even the road and you’d fall in to a deep hole or in a certain tunnel a random wall would appear at the end and you’d run into and it would stay for awhile and I’m playing on Xbox one
