I have a GTX660 with 2GB RAM. The usage of the graphic card is in average at about 60%. For a game with a limit of 30 FPS that should be okay.
The CPU usage seems strange to me, though. I have core i7 3770 at 3,4Ghz (4 physical cores, 4 virtual cores). In average the game uses just about 25% to 26% of my CPU.
I think thats not that much and maybe thats one reason for the problems.
Try run the game windowed, you should see that most of that 25% comes from Core0 and the rest is misc use across other cores. Or check Windows' performance monitor really quick after exiting and you'll see Core0 comes off a far higher curve than the others.
I have a slightly overclocked GTX780 (same GPU use limit before scaling as you, after scaling it's about 80-90%) and then the Haswell version of your processor with the i7-4770. I haven't seen anything like 15 FPS in those areas, maybe a momentary drop to 28 or even 26 but the rest of the time it stays at 30.
Which would then suggest an articial GPU limit because 60% of a GTX780 is a lot more than 60% of a GTX660 and that's the only major difference between our systems.
...then again the lack of change on altering settings suggests a limit outside the GPU...which brings us back to Core0 but we have the same pretty much....
this darn thing doesn't make any sense
using the 60 FPS hack scaling the FPS hitches though would also suggest a limit outside the GPU....

This seems more like a problem with the Multicore support, if it's just using the Core0.
A GPU limit might be more for precautions, to prevent overworking the GPU, as it appears imposed on all, including my GTX560. 60% is usually the standard kind of engineering limit.
Hopefully, this can be patched, or at least modded out. Either or.
