I found out that you'll need to submit a dispute by following the instructions here. Let me know what happens.

    I was banned for a 2nd time for the same name. Problem is, my last ban a few weeks ago was only 3 days. When I went back on the game it doesn't allow me to change it.  Now I'm banned again permanently foe the same name I literally can't change. This is totap BS.
