What happens when the bug occurs? The item equipped on another outfit (home, away, alternate) shows that it's equipped, but it isn't.

When customizing your player in World of Chel, you can't equip the same item to the different home/away/alternate outfits.
For example, let's say I equip a branded goalie blocker A to the home outfit. After equipping blocker A, I save the outfit, switch to another outfit, then try to equip the same blocker A, it shows that blocker A is already equipped. However, once I back out of the item selection menu, blocker A isn't actually equipped. In fact, I can select another blocker (blocker B) and it will show that BOTH blocker A and blocker B are equipped.
The only way I've found around this bug is to select a different branded item, save the outfit, restart the game, then the game will allow me to equip the item I originally wanted.
