  3. Timestamp (date/time, with time zone): june11th/1:44am Central
  4. Which mode were you in?: hockey ultimate team
  5. What were you doing just before it occurred?: trying to change my team lineup 
  6. How often does this issue occur? Is it 100%, every other game, etc?: 50%-99%

    Try creating a new lineup. That has been resolving this for others so hopefully, it works for you as well. Let me know how it goes. Thanks!
