Pressing Home Button on Xbox One causes console to lose video input

I just wanted to submit this issue I've had happen several times now. 

Twice when pressing the Home Button on the Xbox One Controller, the screen will start to flicker for a second and then I lose all video signal on my Xbox One. I can hear sounds and the console is still accepting controller inputs because I can hear the audio. The only way to resolve this is to hold down the power button on the front of the console and do a hard reboot. 

This has only occured while playing this game and I am worried this is going to have some detrimental effect on the Xbox One. Hoping that a patch will resolve this issue in the future because I am not wanting to continue playing the game because I don't want this to have a long term effect on the Xbox.
